Advanced Alternative Medicine Center offers our New Patient forms online so they can be completed in the convenience of your own home or office.

• If you do not already have AdobeReader® installed on your computer, Click Here to download.
• Download the necessary forms, print them out and fill in the required information before coming to the office.
• Bring your printed and completed forms (ALL 5) to your initial appointment.
• Print out the New Patient Coupon by clicking on the previous item in the New Patient Center

1) New Patient Health History Form - Required

This lets us know the history and current state of your health. What questions, concerns, goals, regarding wellness can we help you with? Let us know!  Please fill out ALL four sides being sure to sign the first and last pages before you get to the office.  Thank You.

2) New Patient Appointment Agreement Form - Required

3) New Patient Medication Intake Form - Required

Please fill out this form to include ALL medications and supplements currently being taken, along with past medicines and what they were for. Please bring ALL of your actual medications and supplements with you to your first appointment, as this will help Dr. Huntoon to help you more.

4) New Patient Diet History Form (5 days to be filled out completely) - Required

If you have 5 days before your appointment, please fill this out and bring it with you. If not, please fill it out for 5 days, and then bring it to the office. Thank You.

5) New Patient Questionnaire - Required

Please fill out both sides of the form.  Thank You.

This form can be filled out to register for access to the member wellness section of our website. You can also sign up for our monthly newsletter to keep up on current health issues and news and events in our office.  The online newsletter sign-up is on the upper left of this page, or of the Home Page.
